Students who would like to pay on an installment basis, rather than once at the beginning of each semester, should fill out the Installment Plan License Addendum found below. This form shows the dates and amounts that are due based on the type and term of the license for 2022-2023:

Maryland Law requires all students living in on-campus housing to show proof of having had the meningitis vaccine or to sign a waiver that they are aware that the vaccine is available, know its benefits and have elected not to have the vaccination. The form below will be required upon check-in if University Health Services does not show your proof of having the meningitis vaccination on-file by the time of your check-in to Walker:
The University System of Maryland announced that all students living on a USM campus during the spring 2022 semester must receive an additional dose of the COVID-19 vaccine—a “booster shot”—if they are eligible. Students with approved medical or religious exemptions from the COVID-19 vaccination mandate are not subject to this booster requirement.
Students have two options for reporting their booster:
-If you were vaccinated in Maryland or DC, we encourage you to complete the consent form for CRISP, Maryland’s regional health information exchange, to support collection and use of COVID-19 data important to public health with clinicians and other approved users. You can complete the CRISP consent form here.
-You can also upload your COVID-19 vaccination card here.
